What is the HCD International SAN?
HealthCare Dynamics International (HCDI), a healthcare management and technology consulting firm, along with the National Council of Asian Pacific Islander Physicians (NCAPIP), National Hispanic Medical Association (NHMA), National Medical Association (NMA), and National Minority Quality Forum (NMQF) make up the HCDI SAN.We were awarded a TCPI Support and Alignment Network contract from CMS to identify and assist participating clinicians to meet the initiative’s phases of transformation and associated milestones, with clinical and operational results. Collectively, we represent over 60,000 physicians – 40,200 from primary care and 19,800 from specialties, and a large percentage of medically underserved, urban and rural areas.
Through our partnerships with minority physicians and quality focused organizations comprised of physicians largely serving underserved populations, the HCDI SAN will focus its efforts to improve diabetes care management through a multi-cultural approach to address health disparities. The approach includes a focus on diabetes which has a higher prevalence among certain minority populations; specific education on the collection and use of data stratified by race, ethnicity and language; training in health disparities and cultural and linguistically sensitive delivery of care and performance measurement for diabetes outcomes.
